Hypocalcaemic response to streptomycin in malignant hypercalcaemia.

چکیده
Introduction Malignant hypercalcaemia (pseudohyperparathyroidism) is occasionally associated with primary hepatocellular carcinoma (PHC) or cholangio carcinoma which has not metastasized to bone (Becker, Sternberg and Kalser, 1963; Keller, Goldschneider and Lafferty, 1965; Knill-Jones et al., 1970). Normocalcaemia is restored if the tumour is resected or a liver transplant undertaken (Knill-Jones et al., 1970). Generally, treatment of PHC with radiotherapy or chemotherapy has met with little success and the prognosis remains poor (El-Domeiri et al., 1971; Falkson, 1973). In the case reported here, the primary liver cancer was irresectable. The hypercalcaemia proved refractory to treatment with oral phosphates, glucagon and saline infusions, but responded transiently to intra-arterial mitomycin-C and markedly to parenteral streptomycin. The hypocalcaemic effect of streptomycin has been reported from experimental animals (Galante et al., 1970), but not previously from patients with hypercalcaemia.

Effect of single high dose infusions of aminohydroxypropylidene diphosphonate on hypercalcaemia caused by cancer.
Single intravenous infusions of 30 mg aminohydroxypropylidene diphosphonate were given to 16 patients who had malignant hypercalcaemia to assess host tolerance and the effect on serum calcium concentration. Ten of these patients also received intravenous rehydration or corticosteroids, or both. The serum calcium concentrations decreased significantly after treatment with aminohydroxypropylidene...
متن کاملNormal urine concentrating ability in magnesium depletion.
1. We studied urine concentrating ability in ten magnesium-depleted puppies and compared the results with those of match-fed, littermate controls. 2. The experimental puppies became hypomagnesaemic and hypocalcaemic without evidence of potassium depletion. After 24 h of food and water deprivation, urine osmolality increased to 1350k 340 mosm/kg of water. This value did not differ from that of c...
متن کاملSpontaneous healing of osteitis fibrosa cystica in primary hyperparathyroidism.
A 24-year-old man with primary hyperparathyroidism and osteitis fibrosa cystica developed acute hypocalcaemia. Spontaneous healing of his bone disease was confirmed radiographically and by correction of the serum alkaline phosphatase. Hypercalcaemia associated with a raised serum parathyroid hormone recurred 90 weeks after the initial presentation. During the fourth neck exploration a parathyro...
متن کاملCalcium Disorders in the Emergency Department: Independent Risk Factors for Mortality
BACKGROUND Calcium disorders are common in both intensive care units and in patients with chronic kidney disease and are associated with increased morbidity and mortality. It is unknown whether calcium abnormalities in unselected emergency department admissions have an impact on in-hospital mortality. METHODS This cross-sectional analysis included all admissions to the Emergency Department at...
متن کاملMechanism of malignant hypercalcaemia in carcinoma of the breast.
To investigate the mechanisms of hypercalcaemia in carcinoma of the breast, 22 patients with hypercalcaemia due to metastatic carcinoma were studied and the findings compared with those obtained in normal subjects and patients with benign and malignant breast disease without hypercalcaemia. As expected, patients with metastases of bone showed biochemical evidence of increased bone resorption. W...
